How do I order a Birth, Marriage or Death Certificate?

If the birth, marriage or death occurred in Ontario, you can contact the Head Office of the Registrar General directly at:

Costs are as follows:
Birth certificates (first copy): $25.00
Birth certificates (replacement copy): $35.00
Certified copy: $35.00
Replacement certified copy: $45.00

Payment can be made by Visa or Mastercard; please note that it will be necessary to provide your card number, your complete name and address, and the expiry date of the card. It will take from 6 to 8 weeks to receive the documents you have ordered.

You may also take a cheque to our constituency office made payable to the Ministry of Finance and we will forward your application to the appropriate office.

"Same day" service can be available through the Kingston Land Registry Office (1 Court St., Kingston ON, K7L 2N4) for an additional $15 per certificate. However, this service can only be provided for births since 1930, marriages since 1965 and deaths since 1980. You must bring two pieces of identification.

For a Change of Name application, contact the Thunder Bay office (telephone and fax number shown above) and they will mail it to you.
